Portal Terms of Use

Adaptive SDP, LLC  ·  Effective June 3, 2026

1. Acceptance

By accessing the Adaptive SDP portal, you agree to these terms. If you do not agree, do not use the portal.

2. Authorized Use

The portal is provided exclusively for authorized employees, contractors, and participants of Adaptive SDP, LLC. Access is personal and non-transferable. You must not share your login credentials with any other person.

3. Confidentiality & PHI

The portal contains Protected Health Information (PHI) and confidential organizational data. You are required to maintain the confidentiality of all participant and staff information you access. Accessing records beyond your role-based authorization is strictly prohibited.

4. HIPAA Obligations

All users with access to PHI are bound by HIPAA Privacy and Security Rules. Unauthorized disclosure of PHI may result in:

  • Immediate termination of portal access
  • Civil or criminal penalties under federal law
  • Disciplinary action up to and including employment termination

5. Acceptable Use

  • Do not use the portal for personal or commercial purposes unrelated to Adaptive SDP services.
  • Do not attempt to access systems, accounts, or data beyond your authorized scope.
  • Do not export, copy, or transmit participant or staff data outside of authorized workflows.
  • Do not share screenshots or records of portal content through personal devices or messaging applications.

6. Monitoring

All portal activity is logged. Adaptive SDP, LLC reserves the right to review access logs at any time, and activity may be disclosed to regulatory authorities in the event of a compliance investigation or breach inquiry.

7. Session Security

Sessions automatically expire after 30 minutes of inactivity. You are responsible for logging out when using shared or public devices. Adaptive SDP is not responsible for unauthorized access resulting from failure to log out.

8. Termination of Access

Access will be revoked upon separation from Adaptive SDP, upon expiration of a service authorization, or upon violation of these terms. Continued use of the portal after access has been revoked may constitute unauthorized access under the Computer Fraud and Abuse Act.
